Support Coordination 

Navigating NDIS can be a daunting task. If you are new to the NDIS, making sense of the plan can be overwhelming. Support Coordinators takes that pressure away. 

Support coordination is funded in your plan to support you with building your capacity to understand your plan, connect with service providers and implement your plan to meet your unique needs. PIP Healthcare staff will meet you at a place of your liking. We will help you make sense of the funded plan. Our expert coordinators will listen, share industry knowledge and answer any questions you may have to understand, plan and design your support needs.

The coordinators will use their industry knowledge and experience working with other providers to help find the best service provider to meet your support needs. Being true to our philosophy of ‘No decision about me, without me’, the coordinators will present all the options and if needed facilitate meeting with new providers to help you make an informed decision. The coordinators will do due diligence to ensure you are connected with reputable and values driven service providers. The coordinators can be your sounding board to discuss pros and cons of signing up with a provider. The ultimate decision will always be yours.

Not all services are funded by NDIS. There are many mainstream services that you may not be aware of but are entitled to access them without using the funds in your NDIS plan. Support coordinators can help facilitate referral and establishing connections with mainstream services.

Once decided, the coordinators will facilitate the service agreement, ensuring the terms and conditions are within the limits set by the NDIS. The coordinators can be part of initial meetings with new service providers to ensure you receive quality services. Coordinators will regularly check in with you to ensure services are meeting your needs and money charged for the service are within the limits set by NDIS. At times things may not work out the way you imagined. And that’s fine, coordinators can help facilitate switching over to a new provider. Any issues related to your NDIS plan and services, coordinators can help navigate them and at the same time build your capacity to navigate through such situations by yourself in the future. 

Our Support Coordinators can support you with:

  • Understanding your NDIS plan
  • Plan and design your support needs that meets your unique needs.
  • Engage and connect with service providers funded in your NDIS plan 
  • Engage and connect with mainstream services such as, Centrelink, housing, health services
  • Supports with implementing your support plan
  • Ensure service agreement terms and conditions are within the limits set by NDIS
  • Ensure you are not being over charged for the services being delivered to you
  • Ensure you are receiving quality services that meets your support needs. 
  • Mediate any issues with your service provider
  • Switch to different providers if things are not working out with current service provider. 
  • Life not always goes as planned. Coordinators can help submit Change of Circumstance form if your support needs drastically changes during the current plan period. 
  • Collate all end of plan reports from service providers. 
  • Prepare end of plan report for the NDIS to help facilitate the planning process for the next plan that continues to meet your support needs.
